ACUTE MYOCARDIAL INJURY IN SUBARACHNOID HEMORRHAGE
: TWO CASES REPORTS

ABSTRACT |
The relationship between acute cerebral disorders-namely, spontaneous subarachnoid hemorrhage(SAH), cerebrovascular accident, head trauma, intracranial space-occupying lesions, and meningitis-and various cardiovascular abnormalities has been recognized since the description of Cushing's phenomenon at the turn of the century. Since that time, various authors have reported electrocardiographic( ECG ) aberrations associated with acute intracranial events. The incidental ECG abnormalities in these patients varies between 50% and 90 %.
We present two patients with SAH confirmed on CT scan in whom ECG pattern was simulating myocardial infarction. Two patients have ECG abnormalities in 12-lead surface electrocardiograms after sudden onset of syncopal attack.
In case 1, a 12-lead electrocardiogram reveals elevated ST segment in lead II , III, AVF, V3-6 and depressed ST segment in lead V1.2
In case 2,a 12-lead electrocardiogram reveals elevated ST segment in lead I , AVL and de pressed ST segment in lead II , III, AVF.
