A case of compartment syndrome of the finger due to the accidental injection of a pralidoxime auto-injector

ABSTRACT
A high-pressure injection injury occurs when material is injected into a body part by equipment with sufficient pressure. The authors present a case of a high-pressure injection injury caused by the KMARK-1 kit, resulting in compartment syndrome. A 26-year-old man presented to the emergency department with pain in his right index finger. The symptoms occurred after he was punctured by the KMARK-1 pralidoxime auto-injector during reservist training. Compartment syndrome was suspected, and a fasciotomy was performed three hours later to decompress and release the compartment. The patient completed the treatment without complications four months after the injury. All reported cases of finger high-pressure injection injury due to pralidoxim auto-injectors have been unintentional, and conservative treatment alone has led to recovery. In the present case, compartment syndrome occurred, and skin necrosis progressed after fasciotomy. Therefore, careful evaluation from the outset is necessary in cases of injury caused by pralidoxime auto-injectors.
